Booking and contact details. When you book an appointment or contact us, we collect your name, email address, and phone number so we can schedule and communicate with you. Appointments are managed through Square, our scheduling and point-of-sale provider.

Patient forms.The intake and good faith exam forms collect health information needed to treat you safely, such as allergies, medications, and medical history. Submitted answers are stored on your customer record in our practice management system (Square), and a signed copy of your form is delivered to the practice’s private inbox as the record on file.

Site preferences. If you build a treatment plan on this site, your selections are saved in your own browser (local storage), not on our servers.

We use your information to schedule and provide treatments, communicate about your appointments, keep required medical records, and respond to your questions. We do not sell your personal information, and we do not share it with third parties for their marketing.

We rely on a small number of providers to run the practice: Square (booking, payments, and customer records), our email delivery provider (transmitting signed forms to the practice), and Cherry (optional patient financing, only if you choose to apply through them). Each receives only what it needs to do its job and handles your data under its own privacy policy.

Health details you share through our forms are treated as confidential, are used only for your care, and are accessible only to the practice. Please do not include health information in general contact messages or social media comments.

This site does not use advertising cookies or cross-site trackers. If we measure traffic, we use privacy-friendly, aggregate analytics that do not identify you personally.

Medical records are retained as required by New Jersey law. The site is served over HTTPS, and form submissions are encrypted in transit. No method of transmission or storage is perfectly secure, but we work to protect your information with appropriate safeguards.
